Passport Applications are accepted by appointment only at the Umatilla County Courthouse located at:

216 SE 4th St.
Pendleton, Oregon 97801
Records Office
Room #116

Appointments are available on: Mondays from 12:00 P.M. to 4:00 P.M. and Tuesdays - Fridays from 8:00 A.M. to 4:00 P.M.

All forms & documents are available on the U.S. Department of State website.

* If you are an adult needing to renew your passport, please visit the U.S. Department of State website first, as you may qualify to renew on your own through the mail and will save $35-$50.

Instructions For Filling Out Your Passport Application

Type or Print using BLACK INK ONLY to fill out the application form through Box 21.
DO NOT SIGN THE APPLICATION - EACH APPLICANT MUST APPEAR IN PERSON

Bring the completed application, and the items listed below, with you to your scheduled appointment:

  • A document establishing citizenship:
    • Government issued certified copy of your birth certificate
    • Certificate of Naturalization
    • Certificate of Citizenship
    • Consular Report of a Birth Abroad
    • Previously issued US Passport
      • * Hospital issued birth certificates, US Census Certificates, US Birth Registrations and most Birth Record Abstracts are not acceptable
      • * To establish parental or custodial relationship, an applicant 15 years of age or younger renewing their passport, must also submit a State Certified Copy of their Birth Certificate together with the previously issued US Passport.
  • 1 face-front photograph
    • 2" x 2", Color only with a white background, in which eyes are clearly shown. (No glasses whatsoever, bows, hats or any type of uniform). The County Records Office takes passport photos for $15.00
  • Valid Photo ID
    • Present a valid Driver’s License or other valid government issued photo ID with your signature on it. If your ID is not from Oregon, you will need a secondary piece of ID
  • Children 15 years of age and younger need both parents or legal guardian(s) to appear and sign the application. A Single Parent/Legal Guardian(s) with full Custody needs Court Certified Documentation to submit with each child’s application. Children 16 and 17 need either parent or legal guardian with valid photo ID to appear and sign the application.
  • 1 check or money order per applicant to pay the federal fee. Cash & debit/credit cards are not accepted

Fees

You may apply for a US Passport Book, a US Passport Card, or both.
Please Note: The US Passport Card may only be used for international travel by land or sea between the United States, Canada, Mexico, the Caribbean and Bermuda.

There are 2 separate fees:

  • Cards and Cash are not accepted for the Federal Fee. You must use a Personal Check, Cashier's Check or a Money Order made out to: “US Dept. of State”
  • Card, Cash, Personal Check, Cashier's Check or Money Order made out to: “Umatilla County”
Passport Book Age 16 & Over $130 Plus $35
Passport Book Age 15 & Under $100 Plus $35
Passport Card Age 16 & Over $30 Plus $35
Passport Card Age 15 & Under $15 Plus $35
Passport Book & Card Age 16 & Over $160 Plus $35
Passport Book & Card Age 15 & Under $115 Plus $35

To EXPEDITE any of the following options, add an additional $60 per applicant to the figure in first column. You can also pay an additional $21.36 for overnight return delivery (Passport Book Only).
