The CHART for Umatilla County is committed to supporting healthy communities by cultivating opportunities for citizens to make healthy choices. Our mission is to annually assess the Umatilla County community to evaluate its strengths/resources and weaknesses/needs, then set priorities that will encourage healthy actions by both individuals and communities. An annual reassessment will allow the CHART to gauge progress and update priorities, realizing that nourishing healthy communities must be a long-term commitment shared by individuals and institutions, schools and local governments, private businesses and civic groups. The CHART is committed to building a legacy that will help inspire adults and youth alike to make healthy choices that lead to more fulfilling lives.
Healthy Communities is a population-based approach to
reducing the burden of chronic diseases most closely linked
to physical activity, poor nutrition, and tobacco use –
arthritis, asthma, cancer, diabetes, heart disease, obesity,
and stroke. Through a one-year capacity building grant from
the Oregon State Public Health Division, Umatilla County has
assembled a group of diverse partners who are committed to
conducting a thorough assessment of the county’s
strengths/resources and weaknesses/needs regarding healthy
lifestyles that will then lead to the development of a
three-year plan for improving the health of our citizens.
The Community Health Action and Response Team (CHART) for
Umatilla County is a diverse group of leaders from across
the county representing schools, health care, community
institutions and organizations, and work sites who are
committed to leaving a legacy of improved health
opportunities for the county’s citizens.

According to the Centers for Disease Control and Prevention
(CDC) 7 of the 10 leading causes of death in the United
States are chronic conditions and nearly 50% of Americans
are living with at least one chronic disease. The scope and
impact of chronic disease requires transforming the
environments where people live, work, and play. To that end,
the Community Health Assessment aNd Group Evaluation
(CHANGE) tool was designed to help communities identify and
monitor important policy, systems, and environmental changes
over time. The CHANGE tool is an evidence-based assessment
tool that examines the demographics, physical activity,
nutrition, tobacco, chronic disease management, and
leadership in five sectors that represent our living
environments: community at large, community institution and
organization, health care, school, and work site.

Question Summaries
The Healthy Communities grant year began 7/1/10 and will
end 6/30/11. Data collection will be completed by 2/1/11 and
submitted to the Healthy Communities Coordinator for entry
into the appropriate CHANGE files for submission to the
Oregon Public Health Division by 3/1/11. The three year
Comprehensive Action Plan is due 5/27/11. The Oregon Public
Health Division required three training institutes
throughout the life cycle of this grant:
• Healthy Communities Training Institute I 8/25/10 through
8/26/10
• Healthy Communities Training Institute II 11/12/10 through
11/13/10
• Healthy Communities Training Institute II 3/8/11 through
3/9/11
Each of these institutes provided guidance for the three primary tasks for the grant year: assembling the CHART, using the CHANGE tool, and data analysis and action planning.
